Finally had a chance to get to the bank. Took in the ripped dollar bill I found a few weeks ago.
The nice lady behind the counter informed me that any bill that qualifies for exchange must have
three corners and both serial numbers.
Alas, I had two corners and one serial number......

I chalk that up to 'bank policy' and not Fed policy.
BEP mentions no such requirement, in fact the pieces don't even need to be contiguous, just 50%+
